The Commission of Railway Safety ( CRS ) investigators claimed in the probe report, which was viewed by Reuters, that changes to the signaling circuit to address frequent issues at a nearby rail-road barrier were the cause of the first collision.
It added that the malfunctioning technique directed the traveler train onto the path of the freight train because the local railway staff lacked a common circuit diagram, which caused the boom-barrier circuit to malfunction when they attempted to take it online for repair.
For the first time, Reuters reported last month that investigators were concentrating on the rail-road barrier’s repair job and any potential connections to a regular signaling system bypass.
